Community Nursing
Some support needs a registered nurse. Community nursing brings that clinical layer into the home so hospital and clinic trips are reserved for when they are genuinely necessary.
Funded under Core Supports or Capacity Building, depending on your plan.
What this actually means
Community nursing covers health assessment and monitoring, medication management, wound and catheter care, continence assessment, diabetes support, and clinical care planning alongside your GP and specialists.
It also covers the training and delegation that lets your day-to-day support workers safely carry out nursing-delegated tasks between visits, under proper clinical supervision.
Please note: our community nursing service is currently being restaffed. We are happy to talk with you about your clinical needs, current availability and timing, and to refer you to a trusted local nursing provider in the meantime if that serves you better.
